PLUMBING INSTALLED; PROPER BACKFLOW DEVICES - Comments: OBSERVED A LEAK FROM THE HOT WATER KNOB AT THE THREE COMP SINK IN THE DISHWASHING AREA; INSTRUCTED TO REPAIR LEAK AT FAUCET KNOB. OBSERVED A LEAK FROM THE BOTTOM OF THE ONE COMP SINK NEXT TO THE DISHMACHINE; INSTRUCTED TO SEAL AREA WHERE LEAKING UNDER SINK.

Backflow can contaminate entire water supply with sewage or chemicals.

Plumbing must be properly installed with backflow prevention.

Air gaps at fixtures; Backflow preventers on connections; No cross-connections; Annual testing of devices; Proper drainage.

PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN - Comments: INSTRUCTED TO CLEAN THE FLOOR IN THE KITCHEN AREA UNDER AND BEHIND EQUIPMENT INCLUDING SINKS. CLEAN CEILING VENTS AND TILES IN DISHWASHING AREA TO REMOVE PARTICLES AND DUST OBSERVED. CLEAN FLOOR DRAIN UNDER DISHMACHINE.
