ALSO OBSERVED A WINDOW IN THE KITCHEN COVERED BY CARDBOARD THAT HAS A SCREEN ON IT THAT IS NOT INSECT PROOFED; MUST PROVIDE A SMALLER MESH SCREEN FOR THE WINDOW. OBSERVED SMALL LIVE FLIES (TEN)IN BASEMENT ON WALLS AND SOME EQUIPMENT; MUST ELIMINATE ALL INSECT ACTIVITY. HAVE PREMISES SERVICED BY LICENSED PEST CONTROL COMPANY, WASH AND SANITIZE SURFACES AND EQUIPMENT, COMPLY WITH ALL FOLLOWING ORDERS REGARDING SANITATION AND MAINTAINENCE OF PEMISES. CRITICAL VIOLATION 7-38-020 ISSUED.
