NO EVIDENCE OF RODENT OR INSECT OUTER OPENINGS PROTECTED/RODENT PROOFED, A WRITTEN LOG SHALL BE MAINTAINED AVAILABLE TO THE INSPECTORS - Comments: All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. Over 20 small flies were found in the dining room and utility area, 10 live german cockroaches were found under and around a non-functioning floor drain between the hot water heaters and over a dozen drain flies were found in and around the basement ejector pit area at this time. Remove all evidence of pest activity and clean up all areas of possible harborage. Seal openings and gaps in the walls around the hot water heaters, fill in gaps suport beaqms in said area and clean and maintain ejector pit area clean and dry to reduce insect breeding.
Unprotected openings allow pest entry, leading to contamination of food and surfaces.
Building openings must be protected against pest entry.
Doors tight-fitting and self-closing; Windows screened; Openings sealed; Pest control log maintained; Regular monitoring; Professional service when needed.

THE MIDDLE HOT LINE AND SOUTH WAIT STATION COOLERS HAVE DAMAGED GASKETS THAT NEED TO BE REPLACED. THE AREAS UNDER THE STEAM TABLE, MEAT SLICER CATCH PAN AND DISH TABLE HAVE FOOD AND GREASE BUILDUP WHICH NEEDS TO BE REMOVED.
