NO EVIDENCE OF RODENT OR INSECT OUTER OPENINGS PROTECTED/RODENT PROOFED, A WRITTEN LOG SHALL BE MAINTAINED AVAILABLE TO THE INSPECTORS - Comments: All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OBSERVED 40 FRUIT FLIES IN REAR DISHWASHING/STORAGE AREA ON WALL AND BOXES STORED ON SHELVES. INSTRUCTED TO CONSULT WITH HANSEN PEST CONTROL AND TO ELIMINATE ACTIVITY. OBSERVED 30 MOUSE DROPPINGS IN OUTSIDE STORAGE UNIT WHICH HAS OIL, SODA, CRACKERS, PAPER SUPPLIES AND OTHER ITEMS STORED INSIDE. MUST REMOVE DROPPINGS. RECOMMENDED REMOVING ITEMS OUT OF UNIT TO LOOK FOR HOLES AND IF FOUND TO SEAL THEM. CITATION H000064588 ISSUED FOR IMPROPER PEST CONTROL 7-38-020 (SERIOUS). COURT HEARING ON THURSDAY OCTOBER 21, 2010 AT 400 W. SUPERIOR, ROOM 107 AT 9:00AM.
