ADEQUATE HANDWASHING SINKS PROPERLY SUPPLIED AND ACCESSIBLE - Comments: OBSERVED NO HOT WATER IN ALL STUDENT RESTROOMS ON FLOORS 1,2 AND 3.OBSERVED HOT WATER TEMPERATURE RANGING BETWEEN 55.4 F-79.1F. INSTRUCTED TO PROVIDE HOT WATER TEMPERATURE AT 100F IN ALL RESTROOOMS.

Blocked or unsupplied sinks force employees to skip handwashing, spreading fecal bacteria and viruses to food.

Handwashing sinks must be accessible, properly supplied, and used only for handwashing.

Each sink needs: Warm water (100°F minimum); Soap in dispensers; Paper towels or air dryers; Waste receptacles; Signage; Clear access; Used only for handwashing.

FOOD & NON-FOOD CONTACT SURFACES CLEANABLE, PROPERLY DESIGNED, CONSTRUCTED & USED - Comments: Observed broken seal on walk in cooler #2 door opening. Must repair and maintain same.

Poor design creates areas that harbor bacteria.

All surfaces must be designed to be easily cleanable.

Smooth, non-porous surfaces; Free of cracks and crevices; Easily disassembled; Accessible for cleaning.

PLUMBING INSTALLED; PROPER BACKFLOW DEVICES - Comments: OBSERVED LEAKY AT FAUCET IN REAR KITCHEN AREA. OBSERVED THE FOLLOWING SINKS WITH LOW HOT WATER PRESSURE... EMPLOYEE MENS AND WOMEN WASHROOM,ALL SINKS GIRLS ROOM 109 OBSERVED THE FOLLOWING WASHBOWL SINKS WITH NO WATER... MENS 3RD FLOOR ROOM 301 (3RD AND 4TH SINK ), ROOM 309 GIRLS (3RD AND 4TH SINK), BOYS ROOM 231(2ND AND 4TH SINK) , BOYS ROOM 112 (RIGHT SINK), AND GIRLS ROOM 235 (LEFT SINK). OBSERVED TWO URINALS NOT FLUSHING IN BOYS ROOM 301,AND GIRLS ROOM 305 (1ST TOILET OUT OF ORDER). INSTRCUETD TO REPAIR AND MANITAIN. OBSERVED SEVERAL BROKEN PUSH HANDLES AT WASHBOWLS IN SEVERAL STUDENT RESTROOMS. MUST REPAIR ALL AND MAINTAIN SAME.

Backflow can contaminate entire water supply with sewage or chemicals.

Plumbing must be properly installed with backflow prevention.

Air gaps at fixtures; Backflow preventers on connections; No cross-connections; Annual testing of devices; Proper drainage.

PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN - Comments: Observed rust on ceiling tiles in rear corner of kitchen. Must replace tiles. All surfaces must be smooth and easily cleanable.

Deteriorated facilities harbor pests and prevent proper cleaning.

Building must be maintained in good condition.

No holes in walls/ceilings; Doors and windows intact; Fixtures functional; No water damage; Surfaces intact.
