OBSERVED INADEQUATE/MISSING HANDWASHING FACILITIES IN THE ANNEX BUILDING. OBSERVED TWO CLASSROOMS INSIDE OF THE ANNEX BUILDING. BOTH CLASSROOMS HAVE A RESTROOM WITH A TOILET BUT DO NOT HAVE A HANDWASHING SINK. A SIGN ON THE WALL INSTRUCTED STUDENTS TO USE HAND SANITIZER AFTER USING THE RESTROOM. THE ANNEX BUILDING HAS A MOP SINK IN A ROOM BETWEEN BOTH OF THE CLASSROOMS. SUPERVISOR LOPEZ WAS NOTIFIED. SCHOOL PRINCIPAL WAS NOTIFIED THAT ADEQUATE HANDWASHING FACILITIES (SINK WITH HOT AND COLD RUNNING WATER, HANDSOAP AND HANDTOWELS) MUST BE PROVIDED FOR BOTH RESTROOMS. INSTRUCTED TO USE THE MOP SINK FOR HANDWASHING UNTIL SINKS CAN BE INSTALLED. SERIOUS VIOLATION 7-38-030.

Adequate ventilation required in all areas.
