SEWAGE AND WASTE WATER DISPOSAL, NO BACK SIPHONAGE, CROSS CONNECTION AND/OR BACK FLOW - Comments: OBSERVED THE FACILITY OPEN AND OPERATING WHILE WASTE WATER BACKING UP ONTO THE FLOOR FROM THE FLOOR DRAIN INSIDE OF THE RESTROOM WHEN THE TOILET IS FLUSHED AND WATER IS RELEASED FROM THE WASHBOWL AND WASTE WATER BACKING UP ONTO THE FLOOR FROM THE FLOOR DRAIN WHEN WATER IS RELEASED FROM THE 3 COMPARTMENT SINK AND EXPOSED HAND SINK INSIDE OF THE REAR DRY STORAGE ROOM. INSTRUCTED TO REPAIR AND MAINTAIN ALL PLUMBING, CEASE OPERATIONS AND CLEAN AND SANITIZE ALL SURFACES IN THE ABOVE NOTED AREAS. CRITICAL VIOLATION 7-38-030
