OBSERVED LIVE INSECTS IN THE BASEMENT PREP AREA. OBSERVED APPROXIMATELY 30 LIVE FRUIT FLIES AND DRAIN FLIES ON THE CEILING AND WALLS OF THE BASEMENT PREP AREA, HALLWAY AND RESTROOMS. ALSO OBSERVED STANDING WATER ON THE FLOOR OF THE BASEMENT AROUND THE SUMP PUMP. MANAGEMENT MOPPED UP THE STANDING WATER. SMITHEREEN WAS ONSITE DURING THE INSPECTION. MANAGEMENT INSTRUCTED TO MAINTAIN THE AREA SO AS NOT TO CREATE PEST HARBORAGE AND ELIMINATE THE PEST ACTIVITY. SERIOUS VIOLATION 7-38-020.
