NO EVIDENCE OF RODENT OR INSECT OUTER OPENINGS PROTECTED/RODENT PROOFED, A WRITTEN LOG SHALL BE MAINTAINED AVAILABLE TO THE INSPECTORS - Comments: Outer openings not protected or rodent proofed and pest control logbook not maintained or provided during inspection. Observed fron dual doors with 1/4 inch gap down the middle where doors meet and premises does not have licensed pest control company servicing establishment or written pest control logbook available during inspection. Also, observed 1 baby insect(roach) on back prep table. Instructed to have licensed pest control company service premises, provided written pest control logbook and repair and rodent proof door to prevent rodent and insect entry. Serious citation issued 7-38-020.
