PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN - Comments: 6-501.114 OBSERVED CLUTTER AND ITEMA STORED ON THE FLOOR IN THE TOILET ROOM AND OFFICE. INSTRUCTED TO ORGANIZE, REMOVE UNNECESSARY CLUTTER AND ELEVATE EVERYTHING AT LEAST 6 INCHES FROM THE GROUND.
