NO EVIDENCE OF RODENT OR INSECT OUTER OPENINGS PROTECTED/RODENT PROOFED, A WRITTEN LOG SHALL BE MAINTAINED AVAILABLE TO THE INSPECTORS - Comments: Evidence or rodent activity found inside cluttered closets,gym storage areas. Observed mice droppings-approximately 30 in said cluttered areas,(1)live mouse seen running into an opening beneath the mop sink base. Front entrance double entry doors,rear double doors leading to the garbage areas are not rodent proofed approximately 1/2" mode of entry. Instructed to rodent proof doors to prevent rodent and insect entry. Also, pest control logbook on premises is not complete. Instructed to remove all rodent activity and have licensed pest control service premises and provide logbook with all required documents including a map locator identifying pest control services. Serious citation issued 7-38-020.
