NO EVIDENCE OF RODENT OR INSECT OUTER OPENINGS PROTECTED/RODENT PROOFED, A WRITTEN LOG SHALL BE MAINTAINED AVAILABLE TO THE INSPECTORS - Comments: OBSERVED A 1 INCH GAP AT THE BOTTOM OF THE REAR EXIT DOOR. INSTRUCTED TO INSTALL A PROPER DOOR SWEEP TO PREVENT PEST/RODENT INTRUSION. FOUND 2 DROPPINGS IN A SMALL HOLE NEAR THE FLOOR IN THE BASEMENT DRY STORAGE ROOM NEXT TO THE 2-DOOR REFRIGERATOR. OBSERVED 3 OTHER HOLES IN THE BASEMENT WALL NEAR THE FLOOR IN THE DRY STORAGE ROOM. INSTRUCTED TO SEAL HOLES TO PREVENT HARBORAGE AREAS AND TO PROVIDE A SMOOTH, EASILY CLEANABLE SURFACE. MUST REMOVE DROPPINGS, DETAIL CLEAN, AND SANITIZE AFFECTED AREA. ADDITIONAL PEST CONTROL SERVICE IS NEED TO MINIMIZE OR ELIMINATE ACTIVITY. SERIOUS VIOLATION 7-38-020
