NOTED APPROXIMATELY 7 SMALL BLACK LIVE FLIES FLYING AROUND SODA GUN AND FLOOR DRAINS LOCATED IN BAR PREP AREA, NOTED APPROXIMATELY 15 SMALL BLACK LIVE FLIES FLYING THROUGHOUT REAR PREP/OFFICE AND DISHWASHING AREA NEAR MOP SINK ON WALLS, AND ON TOP OF MOP HEADS. INSTRUCTED TO CONTACT PEST CONTROL FOR SERVICE. MUST ELIMINATE PEST ACTIVITY ON PREMISE. PRIORITY FOUNDATION VIOLATION. # 7-38-020 (A). CITATION ISSUED.

PLUMBING INSTALLED; PROPER BACKFLOW DEVICES - Comments: OBSERVED FAUCET NECK (RIGHT HAND SIDE) ON 3 COMPARTMENT SINK LEAKING. INSTRUCTED TO REPAIR AND MAINTAIN IN GOOD CONDITION.
