NO EVIDENCE OF RODENT OR INSECT OUTER OPENINGS PROTECTED/RODENT PROOFED, A WRITTEN LOG SHALL BE MAINTAINED AVAILABLE TO THE INSPECTORS - Comments: DURING INSPECTION FOUND 5 SMALL FLIES ON WALLS IN THE 2ND SECOND FLOOR PREP AREA ABOVE THE 3-COMP AROUND THE CORNER FROM THE STAFF LADIES RESTROOM. ALSO FOUND APPROXIMATELY 10 LARGE ROACHES AT THE BOTTOM OF THE DOOR FRAME AT THE SAME RESTROOM STATED ABOVE AND AT WALL ABOVE SAME SAID 3-COMP SINK. RECOMMEND PEST CONTROL PROVIDE SERVICE. ABOVE MENTIONED AREAS MUST BE DETAIL CLEANED AND SANITIZED. SERIOUS CITATION ISSUED.
