INSECTS, RODENTS, & ANIMALS NOT PRESENT - Comments: OBSERVED 7 LIVE ROACHES, APPROXIMATELY 4 RAT AND 25 MICE DROPPINGS, SMALL AND BIG FLY ACTIVITY ON SITE. LIVE ROACHES WERE OBSERVED ON THE WALL ABOVE THE HANDWASHING SINK IN THE MAIN PREP AREA, UNDER THE COOKING EQUIPMENT UNDER THE VENTILATION HOOD, IN THE CREVICES OF THE TILE FLOOR NEAR THE WALK-IN COOLER IN THE BASEMENT, ON THE CEILING IN THE BASEMENT, INSIDE THE FRONT FOOD DISPLAY CONTAINER, AND UNDER THE FRONT FOOD DISPLAY CONTAINER, ON THE TABLE IN THE DINING AREA. RAT AND MICE DROPPINGS WERE OBSERVED ON THE FLOOR THROUGOUT THE FACILITY AND ON TOP OF A BULK DRY FOOD CONTAINER IN THE BASEMENT. FLY ACTIVITY WAS OBSERVED IN THE BASEMENT AND IN THE MAIN FOOD PREP AREA. INSTRUCTED TO ELIMINATE LIVE ROACH ACTVITY, REMOVE RODENT DROPPINGS, ELIMINATE FLY ACTIVITY, CLEAN, AND SANITIZE AFFECTED AREAS. ALSO, RECOMMEND CONTACTING PEST CONTROL. PRIORITY FOUNDATION VIOLATION. 7-38-020(A), CITATION ISSUED.
