PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN - Comments: OBSERVED FLOOR IN REAR SODA STORAGE AREA UNDER THE SHELVING WITH A BUILD UP OF DIRT AND FOOD DEBRIS, OBSERVED UNDER THE THREE COMPARTMENT SINK A BUILD UP OF DIRT AND SMALL DEBRIS ALONG THE CORNERS OF THE WALL.. ALSO OBSERVED THE FLOOR IN THE FRONT SERVICE AREA A BUILD UP OF DIRT, DUST AND DEBRIS BETWEEN THE COUNTERS. INSTRUCTED TO CLEAN AND MAINTAIN ALL AREAS.

Building must be maintained in good condition.

No holes in walls/ceilings; Doors and windows intact; Fixtures functional; No water damage; Surfaces intact.
