NO EVIDENCE OF RODENT OR INSECT OUTER OPENINGS PROTECTED/RODENT PROOFED, A WRITTEN LOG SHALL BE MAINTAINED AVAILABLE TO THE INSPECTORS - Comments: OBSERVED LIVE FRUIT FLIES AND A LIVE COCKROACH ON THE PREMISES. 25 LIVE FRUIT FLIES WERE OBSERVED ON THE WALLS OF THE DINING ROOM, BEHIND THE BAR AND ON THE EQUIPMENT IN THE FOOD PREP AREA. 1 LIVE COCKROACH WAS OBSERVED ON THE FLOOR OF THE STORAGE ROOM NEXT TO THE WASHROOMS. ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO MINIMIZE OR ELIMINATE THE PEST ACTIVITY. SERIOUS VIOLATION 7-38-020.
