NO EVIDENCE OF RODENT OR INSECT OUTER OPENINGS PROTECTED/RODENT PROOFED, A WRITTEN LOG SHALL BE MAINTAINED AVAILABLE TO THE INSPECTORS - Comments: OBSERVED EVIDENCE OF RODENT ACTIVITY ON THE PREMISES AND DOORS NOT RODENT PROOFED. OBSERVED APPROXIMATELY 50 RAT DROPPINGS IN THE CABINETS BEHIND THE CAFETERIA SERVICE LINE, IN THE KITCHEN AROUND THE 2 DOOR COOLER AND UNDER THE PAINTED STORAGE CABINETS ON THE FLOOR. ALSO, OBSERVED DOOR #2 NOT RODENT PROOFED AT THE BOTTOM WITH A 1/2" GAP ALONG THE BOTTOM OF THE DOOR. MANAGEMENT INSTRUCTED TO RODENT PROOF ALL EXIT DOORS AND REMOVE ALL RODENT DORPPINGS. ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO MINIMIZE OR ELIMINATE THE PEST ACTIVITY. SERIOUS VIOLATION 7-38-020.
