NO EVIDENCE OF RODENT OR INSECT OUTER OPENINGS PROTECTED/RODENT PROOFED, A WRITTEN LOG SHALL BE MAINTAINED AVAILABLE TO THE INSPECTORS - Comments: OBSERVED APPROXIMATELY 15 RAT DROPPINGS ON THE SIDE OF THE WALK-IN COOLER BETWEEN THE COOLER AND THE EAST WALL OF FACILITY, OBSERVED ONE RAT DROPPING BEHIND THE 2-DOOR REACH-IN COOLER, AND OBSERVED APPROXIMATELY 10 FRUIT FLIES NEAR THE PREP TABLE AND BULK FOOD STORAGE CONTAINERS. ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO MINIMIZE OR ELIMATE RODENT AND PEST ACTIVITY. INSTRUCTED TO REMOVE, DETAIL CLEAN, AND SANITIZE AFFECTED AREAS. OBSERVED A 1 INCH GAP AT THE REAR EXIT DOOR THAT LEADS INTO THE KITCHEN. MUST SEAL THE GAP WITH A PROPER DOOR SWEEP. SERIOUS VIOLATION 7-38-020
