THERE WAS NO SANITIZER ON SITE AT THE TIME OF THE INSPECTION. INSTRUCTED MANAGEMENT ALWAYS TO SET UP ALL THE BASINS FOR THE THREE COMPARTMENT SINK WHEN WASHING DISHWARE. INSECTS, RODENTS, & ANIMALS NOT PRESENT - Comments: OBSERVED APPROXIMATELY 25-30 LIVE SMALL FLIES, 1 LIVE ROACH, AND APPROXIMATELY 5 MICE DROPPINGS ON SITE DURING THE INSPECTION. THE SMALL FLIES WERE OBSERVED FLYING AROUND THE FOOD PREP/DISHWASHING/DRY STORAGE AREA AND ON THE WALLS NEAR THE DISHWASHING MACHINE ESPEICALLY UNDER THE DRAIN BOARD. THE LIVE ROACH WAS OBSERVED ON THE DISHWASHING MACHINE. THE MICE DROPPINGS WERE FOUND ALONG THE FLOORS AND CREVICES IN THE DRY STORAGE AREA. INSTRUCTED TO ELIMINATE SMALL FLY AND LIVE ROACH ACTIVITY. ALSO, REMOVE MICE DROPPINGS, CLEAN AND SANITIZE AFFECTED SURFACES. RECOMMEND CONTACTING PEST CONTROL. PRIORITY FOUNDATION VIOLATION 7-38-020(A), CITATION ISSUED
