(COPY INCLUDED) CHIEF SANITARIAN AND DIRECTOR OF FOOD PROTECTION DIVISION CONSULTED AT THIS TIME. CHARTWELLS SUPERVISOR MS. ARRIVEE TULLIVER WAS CONTACTED AND AGREED TO SEND FRESH SANITIZED UTENSILS WITH THE FOOD DAILY AND TO COLLECT THE DIRTY UTENSILS DAILY UNTIL THE SINK CAN BE INSTALLED OVER THE CHRISTMAS BREAK. IF THE WORK IS NOT COMPLETED BY THE START OF SCHOOL IN JANUARY 2012 THE SCHOOL WILL BE PUT ON ALTERNATIVE FOOD SOURCE BOX LUNCHES PER THE CHICAGO DEPARTMENT OF PUBLIC HEALTH.
