NO EVIDENCE OF RODENT OR INSECT OUTER OPENINGS PROTECTED/RODENT PROOFED, A WRITTEN LOG SHALL BE MAINTAINED AVAILABLE TO THE INSPECTORS - Comments: All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. FOUND 100 MICE DROPPINGS ON THE SHELF WITH CHIPS, 15 ON THE SHELF WITH CAN GOODS IN FRONT OF THE STORE, 25 IN REAR ON SHELF BEHIND MEAT DISPLAY CASE AND 50 LIVE SMALL FLIES IN REAR AROUND 3-COMP. AND MOP SINK. FRONT DOOR NOT RODENT/INSECT PROOFED, 1/4" GAP NOTED. INSTRUCTED TO ELIMINATE ALL ACTIVITY, CLEAN AFFECTED AREAS, MAKE FRONT DOOR TIGHT FITTING AT BOTTOM AND RECOMMENDED CONTACTING PEST CONTROL FOR SERVICES. CITATION #63178-18
