MANAGEMENT PROVIDED HANDSOAP IN THE WASHROOM AT MY REQUEST. MANAGEMENT INSTRUCTED TO PROVIDE SOAP AND PAPER TOWELS AT ALL TIMES AT THE HANDWASHING SINKS. CRITICAL VIOLATION 7-38-030.

OBSERVED 25 MOUSE DROPPINGS ON THE RETAIL AISLE SHELVES WHERE POTATO CHIPS AND SNACKS ARE STORED AND A DEAD MOUSE IN THE GLUEBOARD ON THE SHELF. ALSO, OBSERVED 20 MOUSE DROPPINGS IN THE CABINETS BEHIND THE DELI COUNTER. MANAGEMENT INSTRUCTED TO CLEAN AND REMOVE ALL DROPPPINGS. ALSO OBSERVED LIVE FRUIT FLIES, HOUSE FLIES AND DRAIN FLIES IN THE DELI FOOD PREP AREA, WASHROOM AND BASEMENT. APPROXIMATELY 50 LIVE FLIES WERE OBSERVED IN ALL AREAS. ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO MINIMIZE OR ELIMINATE THE PEST ACTIVITY. SERIOUS VIOLATION 7-38-020.
