VENTILATION: ROOMS AND EQUIPMENT VENTED AS REQUIRED: PLUMBING: INSTALLED AND MAINTAINED - Comments: In boys restroom (east) on 2nd floor, observed faucet timers set between 5-10 seconds for 2 of the 3 faucets on the sink on the left. In boys restroom on 2nd floor (near room 211), observed faucet timers set between 5 seconds - 10 seconds. Observed 1 faucet on the right inoperable. Must repair all. Must set all faucet timers to minimum of 15 seconds for proper hand washing. Must maintain same.
Pests spread numerous diseases through contamination.
No pests or animals allowed in facility.
No evidence of pests; No live animals except service animals; Effective pest control program.
