NO EVIDENCE OF RODENT OR INSECT OUTER OPENINGS PROTECTED/RODENT PROOFED, A WRITTEN LOG SHALL BE MAINTAINED AVAILABLE TO THE INSPECTORS - Comments: OBSERVED APPROXIMATELY 15-20 LIVE FRUIT FLIES ON THE WALLS INSIDE OF THE KITCHEN AND GIRLS RESTROOM AND FLYING AROUND THE KITCHEN. OBSERVED 10 LIVE ANTS CRAWLING ON THE WALL AROUND THE EXPOSED HAND WASHING SINK AND ON THE TOYS (BLOCKS) INSIDE OF THE TODDLERS ROOM. ALSO OBSERVED THE 1/4 INCH OPENING AT THE BOTTOM OF THE FRONT DOOR. OBSERVED CONTAINERS OF INSECT CONTROL CHEMICALS IN THE CLOSET IN THE KITCHEN. INSTRUCTED TO NOT USE THE INSECT CONTROL CHEMICALS AND TO PROVIDE ADDITIONAL PEST CONTROL AND CLEAN AND SANITIZE THE NOTED AREAS TO ELIMINATE THE PEST ACTIVITY AND REMOVE THE OPENING AT THE BOTTOM OF THE FRONT DOOR.
