FOOD PROTECTED DURING STORAGE, PREPARATION, DISPLAY, SERVICE AND TRANSPORTATION - Comments: All food should be properly protected from contamination during storage, preparation, display, service, and transportation. FOOD NOT PROTECTED FROM CONTAMINATION DURING STORAGE. OBSERVED MOLD GROWTH ON INTERIOR CONTACT SURFACES OF THE ICE MACHINE. ALSO OBSERVED RUSTY SCREWS IN CONTACT WITH WATER IN THE RESERVOIR AND A DEAD INSECT FLOATING ON TOP OF THE WATER IN THE RESERVOIR. MACHINE WAS TAGGED HELD FOR INSPECTION. MUST CLEAN AND SANITIZE THE INTERIOR OF THE ICE MACHINE. ALSO OBSERVED LIVE COCKROACHES INSIDE OF THE DRY STORAGE FOOD CONTAINERS OF SUGAR, SALT, BEANS AND BREAD CRUMBS. MANAGEMENT DISCARDED THE CONTAINERS DIRECTLY INTO THE GARBAGE DUMPSTER. SERIOUS VIOLATION 7-38-005A.

NO EVIDENCE OF RODENT OR INSECT OUTER OPENINGS PROTECTED/RODENT PROOFED, A WRITTEN LOG SHALL BE MAINTAINED AVAILABLE TO THE INSPECTORS - Comments: All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OBSERVED LIVE COCKROACHES IN THE DRY STORAGE ROOM IN THE BASEMENT. OBSERVED 15 LIVE COCKROACHES ON THE WALLS, IN THE FOOD STORAGE CONTAINERS AND ON THE STEPS OF THE STAIRCASE. ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO MINIMIZE OR ELIMINATE THE PEST ACTIVITY. SERIOUS VIOLATION 7-38-020.
