TOILET FACILITIES: PROPERLY CONSTRUCTED, SUPPLIED, & CLEANED - Comments: OBSERVED NO WASHROOM AT ABOVE ADDRESS. WASHROOM LOCATED IN ADJACENT RESTAURANT, 3645 1/2 W 83RD PL. EMPLOYEES MUST WALK THROUGH RESTAURANT'S DISH WASHING AND UTENSIL/FOOD STORAGE AREA TO ACCESS WASHROOM. INSTRUCTED MANAGER TO INSTALL ADEQUATE TOILET AND HAND WASHING FACILITIES AT ABOVE ADDRESS.
