NO EVIDENCE OF RODENT OR INSECT OUTER OPENINGS PROTECTED/RODENT PROOFED, A WRITTEN LOG SHALL BE MAINTAINED AVAILABLE TO THE INSPECTORS - Comments: OBSERVED APPROXIMATELY 30 MOUSE DROPPINGS ON THE SHELF UNDER THE PREP TABLE IN THE KITCHEN AND ON THE BOTTOM SHELF OF THE SHELVING UNIT NEXT TO THE BACK DOOR. ALSO OBSERVED A 1 1/2 INCH OPENING AT THE BOTTOM OF THE BACK EXTERIOR DOOR. INSTRUCTED TO CLEAN AND SANITIZE THE AREAS WITH MOUSE DROPPINGS. PROVIDE ADDITIONAL PEST CONTROL TO ELIMINATE THE PEST ACTIVITY AND TO SEAL THE OPENING AT THE BACK DOOR. SERIOUS VIOLATION 7-38-020
