OBSERVED BLACK SLIME SUBSTANCE IN UPPER PANEL WATER RESERVOIR OF ICE MACHINE WHERE ICE IS MADE AND DRIPPING INTO ICE IN BIN. INSTRUCTED TO DISCARD ALL ICE. MACHINE TAGGED HELD FOR INSPECTION AND MUST NOT USE UNTIL CLEANED, SANITIZED AND C.D.P.H. NOTIFIED FOR INSPECTION AND TAG REMOVAL.

OBSERVED 6 RAT DROPPINGS IN COAT CHECK ROOM AND OPEN GAP ALONG BASEBOARD IN SAME ROOM THAT APPEARS TO LEAD UNDER MAIN FLOOR OF ESTABLISHMENT WITH OPEN EARTH. MUST REMOVE ALL DROPPINGS AND DEBRIS ON FLOOR IN ROOM, SEAL OPEN GAP ALONG BASEBOARD AND HAVE PEST CONTROL SERVICE ESTABLISHMENT FOR RODENTS AND ROACHES. ALSO OBSERVED TWO LARGE, LIVE ORIENTAL ROACHES ON WALL IN BASEMENT.
