All hand sinks must be maintained, working and easily accessible at all times. Operator connected hand sink during inspection. Hand sink is functioning properly. Critical violation 7-38-030.

NO EVIDENCE OF RODENT OR INSECT OUTER OPENINGS PROTECTED/RODENT PROOFED, A WRITTEN LOG SHALL BE MAINTAINED AVAILABLE TO THE INSPECTORS - Comments: Observed approximately 50 total mice droppings in the following areas: On pipe in corner of wall and on the floor of storage room in multi purpose room; in room 108 in storage closet; in room 106 on the floor of closet, and on the floor in corner next to door that leads to room 104; in room 104 on the floor behind the mini fridge, on the floor in the closet, under the washbowl sink, and under the toilet. Must remove all droppings, clean and sanitize all affected area. Advised to call pest control for service. Must eliminate pest issue. Serious violation 7-38-020.

Unprotected openings allow pest entry, leading to contamination of food and surfaces.

Building openings must be protected against pest entry.

FLOORS: CONSTRUCTED PER CODE, CLEANED, GOOD REPAIR, COVING INSTALLED, DUST-LESS CLEANING METHODS USED - Comments: Observed excessive clutter and dust build up on floors of teachers' storage closet on the first floor of old building (rooms 104, 106, and 108). Must clean floors and organize excessive clutter to prevent pest harborage. Must elevate all items at least 6 inches above the floor to allow for proper cleaning. Must maintain same.

Damaged floors harbor bacteria and create safety hazards.

Floors must be smooth, cleanable, and maintained.

Smooth, non-porous in food areas; Properly sloped to drains; Coving at wall junctures; Kept clean; Good repair.

WALLS, CEILINGS, ATTACHED EQUIPMENT CONSTRUCTED PER CODE: GOOD REPAIR, SURFACES CLEAN AND DUST-LESS CLEANING METHODS - Comments: Observed holes in walls around plumbing under washbowl sinks and toilets in rooms 104, 106, and 108. Must seal all gaps and holes to prevent pest entry. Observed gaps in walls around plumbing in girls and boys restrooms on 2nd floor in old building. Must seal all holes to prevent pest entry. Observed large gap around wall pipe in multi purpose storage room. Must seal to prevent pest entry. Must maintain same.
