PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN - Comments: OBSERVED DIRTY FLOOR WITH DEBRIS INSIDE HOT WATER HEATER STORAGE CLOSET LOCATED ON THE SECOND FLOOR. INSTRUCTED TO DETAIL CLEAN FLOOR AND MAINTAIN.

Deteriorated facilities harbor pests and prevent proper cleaning.

Building must be maintained in good condition.

ADEQUATE VENTILATION & LIGHTING; DESIGNATED AREAS USED - Comments: OBSERVED NON-WORKING LIGHT BULB INSIDE THE MILK REFRIGERATOR LOCATED ON THE 1ST FLOOR TEACHERS' BREAK ROOM. INSTRUCTED TO REPLACE AND MAINTAIN ADEQUATE LIGHTING AT ALL TIMES.
