PREVIOUS SERIOUS VIOLATION FROM REPORT #612549 ON 11/7/11 NOT CORRECTED: 18/01- All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OBSERVED LIVE FRUIT FLIES ON THE PREMISES . OBSERVED APPROXIMATELY 35 LIVE FRUIT FLIES ON THE WALLS, CEILING AND EQUIPMENT OF THE PREP AREA AND JUICE BAR SEATING AREA. SUPERVISOR LOPEZ WAS NOTIFIED. TECHNICIANS FROM MARK'S PEST CONTROL ARRIVED DURING THE INSPECTION FOR A PEST CONTROL INSPECTION. STAFF USED A VACUUM TO ELIMINATE FRUIT FLIES IN THE AREAS MENTIONED ABOVE. MUST CONTINUE TO CLEAN FLOOR DRAINS AND MAINTAIN A CLEANING SCHEDULE ALONG WITH REGULAR PEST CONTROL SERVICE TO MINIMIZE THE PEST ACTIVITY. CRITICAL VIOLATION 7-42-090.
