NO EVIDENCE OF RODENT OR INSECT OUTER OPENINGS PROTECTED/RODENT PROOFED, A WRITTEN LOG SHALL BE MAINTAINED AVAILABLE TO THE INSPECTORS - Comments: All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OBSERVED LIVE FRUIT FLIES ON THE PREMISES AND IMPROPER PEST CONTROL. OBSERVED APPROXIMATELY 20 LIVE FRUIT FLIES ON THE WALLS, CEILING AND EQUIPMENT OF THE PREP AREA AND JUICE BAR SEATING AREA. 5 FLIES WERE OBSERVED IN THE PREP AREA AND ABOUT 15 IN THE SEATING AREA. 2 FLY STRIPS WERE OBSERVED IN THE PREP AREA IN THE DRY STORAGE HALLWAY AND ABOVE THE 3 COMPARTMENT SINK. MANAGEMENT REMOVED THE FLY STRIPS FROM THE CEILING. ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO MINIMIZE OR ELIMINATE THE PEST ACTIVITY. SERIOUS VIOLATION 7-38-020.
