NO EVIDENCE OF RODENT OR INSECT OUTER OPENINGS PROTECTED/RODENT PROOFED, A WRITTEN LOG SHALL BE MAINTAINED AVAILABLE TO THE INSPECTORS - Comments: FOUND ABOUT 50 MOUSE DROPPINGS ALONG BRICK WALL BASE, ALONG WINDOW LEDGES, AND AROUND PLAY LOT EQUIPMENT THROUGH OUT SEATING/PLAY LOT AREA. ALSO, FOUND 1/4TH GAP AT RIGHT BOTTOM SIDE OF EXIT DOOR. INSTRUCTED TO CLEAN, SANITIZE AFFECTED AREAS, RECOMMEND PEST CONTROL TO SERVICE PREMISES, AND RODENT PROOF SAID DOOR TO MAKE TIGHT FITTING TO PREVENT RODENT ENTRY. SERIOUS 7-38-020 ISSUED.
