NO EVIDENCE OF RODENT OR INSECT OUTER OPENINGS PROTECTED/RODENT PROOFED, A WRITTEN LOG SHALL BE MAINTAINED AVAILABLE TO THE INSPECTORS - Comments: Evidence or rodent activity found on the premises. Observed mice droppings-approximately 30 behind cluttered 2nd flr main building boiler- room throughout . Front entry door has 1/4" inch gap bottom left corner to boiler room. Instructed to rodent proof door to prevent rodent and insect entry. Also, establishment does have pest control logbook on premises. Instructed to remove all rodent activity, seal noticeable openings on floor same boiler room and have licensed pest control service the premises,remove clutter due to pest harborage. Engineer is made aware of said conditions during this inspection. Serious citation issued 7-38-020.
