NO EVIDENCE OF RODENT OR INSECT OUTER OPENINGS PROTECTED/RODENT PROOFED, A WRITTEN LOG SHALL BE MAINTAINED AVAILABLE TO THE INSPECTORS - Comments: OBSERVED THE FRONT EXTERIOR DOOR GLASS MISSING AND PLASTIC PARTIALLY TAPED TO COVER THE OPENING. ALSO OBSERVED THE REAR EXTERIOR DOOR WITH A 1/4 INCH OPENING AT THE BOTTOM. APPROXIMATELY 6-8 LARGE FLIES FLYING AROUND THE FACILITY. NO PEST CONTROL LOG ON SITE AND AVAILABLE DURING THE INSPECTION.
