NO EVIDENCE OF RODENT OR INSECT OUTER OPENINGS PROTECTED/RODENT PROOFED, A WRITTEN LOG SHALL BE MAINTAINED AVAILABLE TO THE INSPECTORS - Comments: OBSERVED THE FRONT EXTERIOR DOOR GLASS MISSING AND PLASTIC PARTIALLY TAPED TO COVER THE OPENING. ALSO OBSERVED THE REAR EXTERIOR DOOR WITH A 1/4 INCH OPENING AT THE BOTTOM. APPROXIMATELY 6-8 LARGE FLIES FLYING AROUND THE FACILITY. NO PEST CONTROL LOG ON SITE AND AVAILABLE DURING THE INSPECTION. INSTRUCTED TO REPAIR DOORS SO THAT THEY ARE COMPLETELY RODENT PROOF. PROVIDE ADDITIONAL PEST CONTROL TO ELIMINATE THE FLY ACTIVITY AND PROVIDE A PEST CONTROL LOG ON SITE. SERIOUS VIOLATION 7-38-020
