Observed female and clssroom of students inside first floor toilet room, enter and exit toilet room without having liquid hand soap and also no liquid hand soap provided inside toilet rooms. Also, only one set of toilet rooms are operable for students use. Toilet rooms are pad locked on second floor and west end of building. Instructed to detail clean, repair all non-working facilities, remove unnecessary articles and have accessible to students. Critical citation issued 7-42-090.

Second floor east fire escape has gap in bottom corner left side. Instructed to rodent proof door to prevent insect and rodent entry. Serious citation issued 7-38-020.
