PREVIOUS SERIOUS VIOLATION CORRECTED, 7-42-090 - Comments: Previous serious citation not corrected: 05-07-2012, report #1214664: Toilet rooms not cleaned or provided with hand cleaner. Observed female and clssroom of students inside first floor toilet room, enter and exit toilet room without having liquid hand soap and also no liquid hand soap provided inside toilet rooms. Also, only one set of toilet rooms are operable for students use. Toilet rooms are pad locked on second floor and west end of building. Instructed to detail clean, repair all non-working facilities, remove unnecessary articles and have accessible to students. Critical citation issued 7-42-090.

NO EVIDENCE OF RODENT OR INSECT OUTER OPENINGS PROTECTED/RODENT PROOFED, A WRITTEN LOG SHALL BE MAINTAINED AVAILABLE TO THE INSPECTORS - Comments: Outer openings not protected or rodent proofed. Second floor east fire escape has gap in bottom corner left side. Instructed to rodent proof door to prevent insect and rodent entry. Serious citation issued 7-38-020.
