INSECTS, RODENTS, & ANIMALS NOT PRESENT - Comments: OBSERVED A STICKY TRAP UNDER THE THREE COMPARTMENT SINK WITH BUGS. INSTRUCTED TO REPLACE SAID STICKY TRAP TO MONITOR ACTIVITY AND MAINTAIN.

UTENSILS, EQUIPMENT & LINENS: PROPERLY STORED, DRIED, & HANDLED - Comments: OBSERVED THE KITCHENWARE ON THE WIRE RACK UNDER THE FOOD PREP TABLE NOT INVERTED. INSTRUCTED TO INVERT SAID KITCHENWARE AND MAINTAIN.

Facility must be free of unnecessary items and litter.

Remove unnecessary equipment; No litter; Organized storage; No living quarters; Personal items in designated areas.

PHYSICAL FACILITIES INSTALLED, MAINTAINED & CLEAN - Comments: OBSERVED THE FLOORS UNDER THE HANDWASHING SINK AND IN THE INSIDE OF THE METAL PLATFORM UNDER THE HOT WATER TANK WITH SLIGHT DEBRIS; BOTH MENTIONED AREAS WERE OBSERVED IN THE FOOD PREP AREA. INSTRUCTED TO CLEAN AND MAINTAIN SAID FLOORS.
