ALSO, OBSERVED NO SOAP IN THE SOAP DISPENSER IN THE FOLLOWING WASHROOMS: BASEMENT BOY'S WASHROOM, 2ND FLOOR BOY'S AND GIRL'S WAHSROOMS AND 1ST FLOOR BOY'S AND GIRL'S WASHROOMS. MANAGEMENT PROVIDED SOAP IN ALL WASHROOMS AT MY REQUEST. MANAGEMENT INSTRUCTED TO MAINTAIN ALL WASHROOMS CLEAN AT ALL TIMES AND TO PROVIDE HAND SOAP IN ALL WASHROOMS AT ALL TIMES. SERIOUS VIOLATION 7-38-030.
