NO EVIDENCE OF RODENT OR INSECT OUTER OPENINGS PROTECTED/RODENT PROOFED, A WRITTEN LOG SHALL BE MAINTAINED AVAILABLE TO THE INSPECTORS - Comments: All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OBSERVED THE FOLLOWING: NINE RAT DROPPINGS IN MIDDLE BASEMENT BEHIND BOX FREEZER, SIX DROPPINGS BEHIND WATER HEATER, TEN DROPPINGS ALONG BASEBOARDS THROUGHOUT DRY STORAGE SIDE OF BASEMENT. ALSO OBSERVED APPROXIMATLY 15 TO 20 FRUIT FLIES ON PLASTIC GARBAGE CAN IN BASEMENT, 10 TO 15 FLIES AROUND THREE COMPARTMENT SINK IN BAR. WOODEN FOLDING WINDOW BETWEEN COOKS LINE AND OUTDOOR PATIO NOT INSECT PROOFED. OPEN 1/2 INCH GAP ON ALL SIDES AND SEAMS OF FOLDING WINDOW. PARTIAL GLASS MISSING. MUST MAKE WINDOW TIGHT FITTING ON ALL SIDES, REMOVE ALL DROPPINGS, SANITIZE ALL AREAS AND MINIMIZE OR ELIMINATE FLY ACTIVITY. PEST CONTROL ARRIVED TO TREAT ESTABLISHMENT DURING INSPECTION.

Doors tight-fitting and self-closing; Windows screened; Openings sealed; Pest control log maintained; Regular monitoring; Professional service when needed.
