SECOND FLOOR BOY'S WASHROOM NO SOAP NO PAPER TOWELS, NEW BUILDING. OLD BUILDING MEN'S WASHROOM NO SOAP, NO HAND DRYING DEVICE PROVIDED, ACROSS FROM ROOM #111. GIRL'S WASHROOM AT OLD BUILDING ACROSS FROM ROOM #115 NO SOAP. OBSERVED ONE TEACHER PROVIDING SOAP TO CHILDREN, HOWEVER NO PAPER TOWEL WAS PROVIDED AT WASHROOM AND CHILDREN DRIED THEIR HANDS ON UNIFORM. ANOTHER TEACHER AT FIRST FLOOR PROVIDED SANITIZING SOLUTION AFTER THE CHILDREN USED THE WASHROOM, SOME THE CHILDREN WASHED THEIR HANDS ONLY WITH WATER, SOME OF THE CHILDREN DID NOT WASH HANDS, AFTER EACH CHILD GOT SANITIZING SOLUTION TEACHER PROVIDE THEM WITH TOILET TISSUE PAPER WHICH MUST OF THE PAPER GOT STUCK ON THEIR HANDS. INFORMATION ON HAND WASHING WAS GIVEN TO CERTIFIED FOOD HANDLER. :#29- PREVIOUS MINOR VIOLATION NOT CORRECTED FROM 3-26-2012, REPORT #679817 #32-REMOVE EXCESS RUST AND PAINT RADIATOR IN MEN'S WASHROOM ABOVE THE URINALS, THIRD FLOOR(NORTH SIDE). #34-REPLACE MISSING FLOOR TILES IN MEN'S WASHROOM, AT THIRD FLOOR REPAIR LEAKING FAUCET AT 3 COMPARTMENT SINK, FIRST COMPARTMENT SINK BY THE OVENS.
