HAND WASHING FACILITIES: WITH SOAP AND SANITARY HAND DRYING DEVICES, CONVENIENT AND ACCESSIBLE TO FOOD PREP AREA - Comments: OBSERVED THE ELECTRIC HAND DRYERS IN THE 3RD FLOOR GIRL'S WASHROOM AND 2ND FLOOR GIRL'S WASHROOM NOT OPERATIONAL. ENGINEER PROVIDED PAPER TOWELS FOR THESE WASHROOMS. ALSO, OBSERVED NO HAND SOAP IN THE 2ND AND 3RD FLOOR BOY'S WASHROOMS. ENGINEER PROVIDED HAND SOAP AT MY REQUEST. MANAGEMENT INSTRUCTED THAT SOAP AND SANITARY HAND DRYING DEVICES MUST BE PROVIDED IN ALL WASHROOMS AT ALL TIMES. CRITICAL VIOLATION 7-38-030.

OBSERVED DOORS #4 AND #5 NOT RODENT PROOFED WITH A 1/2" GAP ALONG THE BOTTOM OF THE DOORS. MUST RODENT PROOF ALL OUTER OPENINGS. ALSO, OBSERVED APPROXIMATELY 50 MOUSE DROPPINGS ON THE FLOOR OF THE DRY STORAGE ROOM AND BEHIND THE OVEN NEXT TO THE WALK-IN FREEZER. MANAGEMENT INSTRUCTED TO CLEAN AND SANITIZE ALL AFFECTED AREAS AND REMOVE DROPPINGS FROM ALL AREAS. ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO MINIMIZE OR ELIMINATE THE PEST ACTIVITY.
