NO EVIDENCE OF RODENT OR INSECT OUTER OPENINGS PROTECTED/RODENT PROOFED, A WRITTEN LOG SHALL BE MAINTAINED AVAILABLE TO THE INSPECTORS - Comments: OBSERVED 20 MICE DROPPINGS IN THE PRODUCE REAR PREP AREA, COMPOSE AREA. ALSO OBSERVED 20 MICE DROPPINGS IN THE DELI AREA BEHIND THE RECYCLE BINS, AND UNDER THE DISH MACHINE. DID FIND SEVERAL HOLES IN THE WALL IN IN CORNER BY COMPOSE BINS PRODUCE AREA AND IN DELI AREA HOLE IN WALL BY PROOFER, AND BROKEN TILE/HOLE IN FRONT DELI WALL END OF CHEESE CASE. INSTRUCTED TO REMOVE ALL DROPPINGS SANITIZE ALL AFFECTED AREAS. HAVE PEST CONTROL RETURN FOR SERVICE. ALSO OBSERVED THE FRONT CART DOOR NOT RODENT PROOFED 1/2" GAP UNDER DOOR(BROKEN CONCRETE/NEEDS REPAIR). SERIOUS VIOLATION 7-38-020.
