AT THIS RE-INSPECTION OF OFFICE ASSIGNMENT STATUS THE FACILITY WAS STILL FOUND WITH A SUBSTANTIAL AMOUNT OF SMALL FLIES AND NOT MINIMIZED. 15 LIVE FLIES FOUND THROUGHOUT THE DELI PREP, 3 IN THE MEAT PREP, 26 ON WALLS AND THROUGHOUT THE PRODUCE STORAGE AND PREP, 11 IN THE MOP ROOM AND AROUND THE TRASH COMPACTOR AREA, 4 IN THE EMPLOYEE WASHROOMS. DIRTY FLOORS WITH FOOD DEBRIS FOUND IN THE DELI PREP, BAKERY PREP, IN EMPLOYEE LOUNGE UNDER STORAGE LOCKERS. DIRTY FLOOR DRAINS FOUND IN DELI AND BAKERY PREP.PLASTIC CRATES USED AS SHELVING IN THE BAKERY FOUND DIRTY. SEVERAL TRASH CONTAINERS FOUND DIRTY IN DELI AND EMPLOYEE LOUNGE. NO CITATION ISSUED AT THIS TIME. ALL AREAS MENTIONED ABOVE REMAIN CLOSED AND MUST NOT USE UNTIL RE-OPENED BY CDPH. MUST CONTACT CDPH BY FAX OR EMAIL REQUESTING RE-INSPECTION AND RE-OPENING WITH AN UPDATED PEST CONTROL SERVICE REPORT.
