NO EVIDENCE OF RODENT OR INSECT OUTER OPENINGS PROTECTED/RODENT PROOFED, A WRITTEN LOG SHALL BE MAINTAINED AVAILABLE TO THE INSPECTORS - Comments: Found approximately 15 small black flies in front server station, 3 in rear dining room, and 5 in dish area of kitchen. Instructed to detail clean and sanitize these areas. Recommended to call pest control for service to eliminate small black flies. Found approximately 2 inch gap in bottom right corner of rear outside entry door. Must repair. Found 1 inch gap along lower left side of front (east side) entry door. Must repair. All doors must be tight fitting to prevent pest entry. Must maintain all. Serious violation 7-38-020.
