Observed several students in annex building going and coming from toilet room without proper hand washing. Inside toilet rooms not provided with liquid soap or paper towel. Engineer stated that teachers provided soap and paper towel for students. However, both students were without soap or paper towel before and after entering toilet room. Also, no written policy is available or posted in regards to hand washing procedure. Instructed to have written policy available and/or posted and to monitor children for proper hand washing.
