UPDATED COPY OF THE IDPH LICENSE,MAP LOCATOR INDICATING THE PEST CONTROL SERVICES PROVIDED FOR THIS MFD TRUCK.ALSO,THE SERVICE WINDOWS MUST BE SCREENED AND THE DRIVER/PASSENGER DOORS TO THE MFD TRUCK AT CORNERS MUST BE RODENT PROOF TO PREVENT PEST ENTRY.
