OBSERVED AVER 20 LIVE FRUIT FLIES ON WALL IN REAR FOOD STORAGE ROOMS AND INSIDE WASHROOMS. MUST CLEAN AREAS. OBSERVED FRONT ENTRANCE DOOR NOT RODENT PROOF DOOR HAS APPROXIMATELY ONE INCH OPENING ON BOTTOM DOOR. MUST MAKE DOOR RODENT PROOF TO PREVENT PEST ENTRY. SERIOUS VIOLATION 7-38-020.
