INSTRUCTED TO REPAIR LEAKS AND MAINTAIN ALL PLUMBING. TODAY 4-30-15 OBSERVED THE SAME VIOLATIONS,EXCESS WATER ON FLOOR OF THE BASEMENT,UNABLE TO CHECK ONE ROOM DUE TO WATER,WATER IS RUSTY.A STAFF:ENGINEER AND HOUSEKEEPING EMPLOYEE DINING AREA ARE LOCATED AT THIS AREA. SERIOUS VIOLATION:7-42-090.NO CITATION ISSUED AN EXTENSION WAS REQUESTED FOR THIS VIOLATION UNTIL JUNE 18,2015,WHICH WAS GRANTED BY THE PUBLIC HEALTH
