NO EVIDENCE OF RODENT OR INSECT OUTER OPENINGS PROTECTED/RODENT PROOFED, A WRITTEN LOG SHALL BE MAINTAINED AVAILABLE TO THE INSPECTORS - Comments: OBSERVED 50 SMALL FLIES AND 10 LARGE FLIES IN VARIOUS AREAS OF THE FACILITY; 30 SMALL FLIES IN THE WOMEN'S RESTROOM, 20 SMALL FLIES AROUND THE MOP SINK, 10 SMALL AND LARGE FLIES IN THE REAR MEAT PREP AREA. OBSERVED BOTH REAR DOORS OPEN WITH SCREEN DOORS THAT ARE NOT TIGHT FITTING FOR DOOR SIZE. OBSERVED A 6 INCH GAP AT THE TOP OF SAID DOORS. INSTRUCTED TO KEEP MAIN REAR DOORS CLOSED AT ALL TIMES UNTIL SCREEN DOORS ARE INSTALLED THAT ARE TIGHT FITTING. INSTRUCTED TO REMOVE FLIES, CLEAN AND SANITIZE AFFECTED AREAS. RECOMMENDED CONTACTING PEST CONTROL FOR SERVICE. SERIOUS VIOLATION 7-38-020.
